*Beginning Jan. 1, 2026, Bariatric surgery will only be covered by the Plan if it is performed by a Lantern surgeon. All Lantern bariatric surgeons are members of ASMBS (American Society of Metabolic and Bariatric Surgery). Lantern surgeons are held to the well-defined and rigorous standards of ASMBS including preop assessment and medical optimization.

What if I haven't received a card or need a replacement card?
